There are two different ways to create a team:
1. Click on the Team Registration Form link on the Downloads page to download a copy of the team registration form.
After filling it out you can turn it into the SIAC office upstairs in the Union by October 23rd.
2. Click on the Team Registration Form link on the Downloads page to download a copy of the team registration form. Then
email is to us at
unistjude@gmail.com attn: Amy by October 23rd.
**We prefer that you email the forms to us so it is easier to read/spell your names and emails correctly. And it saves
paper!**
